- Comprehensive review of all current medications and supplements
- Assessment of mobility, balance, and fall risk factors
- Cognitive and memory screening using structured, validated tools
- Discussion of any changes in daily function, sleep, appetite, or mood
- Review of advance directives and care preferences if desired
- Coordination with family members or caregivers when appropriate

- Medicare covers cognitive assessments and advance care planning conversations
- Bring all medications in their original bottles — including over-the-counter and herbal supplements
- Family members or caregivers are welcome to join the appointment
- We can provide documentation for advance directives and POLST forms
- All visits are available in Korean — no need to bring a translator
